Change Your Name
Filing a Name Change Petition
Petitions for name change are filed with the District Court and require two forms: Petition for Changing Name and Order for Changing Name. Please complete the interview process for a Name Change Petition online (this is an electronic filing). Click here to view the Name Change Information Packet.

If the name change request is for a minor child, each parent must sign an affidavit indicating that they both agree and consent to the change.
Note: The Name Change Information Packet is in a PDF format. In order to view the file, you must have Adobe Acrobat Reader installed on your system. You can download the Adobe Acrobat Reader software at no cost; OR complete the interview process for a name change petition online.
After you submit the petition online, you will be given a confirmation number. You must appear at the Pierce County District Court (2nd floor) within 72hrs for review and to file the Name Change.
Please bring the following items to your scheduled appointment:
- Official photo ID (driver's license, passport, etc.)
- Proof of residency
- If the petitioner has a felony on their record, they must provide proof from the Department of Corrections that the issue has been resolved.
- If minor (under age 18), provide an original, certified birth certificate and official ID (if available)
- If minor (under age 18), provide signed affidavits from both parents, for each minor
Upon filing the case, you will be given instructions to pay the filing fee and a court date will be scheduled. All adults and minors over the age of 10, must appear for court.
